    Hospice is patient centered care provided in the home setting when you might be facing a terminal diagnosis. The hospice can come to you anywhere that you call home, whether that be in your house, an assisted living, skilled nursing home or board and care. We are there to provide those services for you and prioritize your comfort. This high level of outpatient excellence in care is achieved by bridging together a team of passionate and compassionate professionals with the same goal in mind, your comfort. The team consists of doctors, nurses, pharmacists, bath aides, spiritual counselors, social workers, volunteers, and the list goes on. Patients will have regularly scheduled visits and in addition to those regularly scheduled visits, you can call us 24 hours a day, seven days a week, and we will be available to you to address any concerns. You can kind of think of us as a doctor’s office on wheels. We are here for resources and anything else you might need. The biggest tool that we have are medications and we use these medications to make sure that you’re comfortable. But in addition, we also provide things like supplies, equipment and any type of procedures that might be necessary and that are able to be done in the home setting. The idea is that you want to avoid hospitalization and we want to help you do that. You can call us 24 hours a day, 7 days a week for anything. This program is covered by most insurances, including Medicare, Medical, and most private insurances. You can kind of think of hospice as a big hug at a really difficult time. We’re here for you to make sure that this journey is as painless as possible.
